What Is the Medicare Prescription Payment Plan?

The Medicare Prescription Payment Plan is a payment option for people with Medicare drug coverage. It lets you spread covered Part D out-of-pocket costs across the calendar year instead of paying the full amount at the pharmacy when a prescription is filled.

What the plan does

You still owe the cost of your covered prescriptions. The payment plan does not reduce the price of a drug, remove the plan’s rules, or guarantee that your monthly bill will stay the same. A plan calculates monthly payments using your costs and the months remaining in the year.

Who may find it useful

It may help people who face high prescription costs early in the year and prefer predictable monthly bills. It may not lower total costs, and it may not be the best fit for everyone.

Before you choose it

  • Ask your plan how to participate and how bills are calculated.
  • Compare the payment option with programs that may lower costs, such as Extra Help.
  • Keep paying each monthly bill after enrolling.
  • Ask what happens if you change plans, stop coverage, or miss a payment.

The annual out-of-pocket limit for covered Part D drugs is year-specific. Medicare.gov lists $2,100 for 2026. Confirm current figures and rules at Medicare.gov.
